HOW TO ORDER

Ordering Procedures and Protocols:

A request form needs to be completed, signed and dated by both the Practitioner and patient.

OncoDNA also require a separate Patient Informed Consent Form be completed by the patient only.

As OncoDNA provide several analyses using solid, liquid and a combination of both biopsies, the associated logistics vary slightly. However, regardless of the analysis ordered, there is minimal effort required on behalf of both the Practitioner and/or patient.

Where a solid biopsy is necessary, we require:

  1. Request Form

  2. OncoDNA Patient Informed Consent Form

  3. Copy of the histopathology report

Genostics will then organise retrieval of the specimen from the relevant pathology lab. Following receipt of the solid sample, we will then express ship the sample to the laboratory in Belgium for analysis.

Where a liquid biopsy is necessary, we require:

  1. Request Form

  2. OncoDNA Patient Informed Consent Form

Genostics will then send a blood collection kit to the patient for standard blood collection (20ml) at any local pathology. The kit contains all pre-filled documents and instructions for shipment of the sample to Belgium for analysis.
